As an employee of the fire service, this form is for you to use to give information on wrongdoing by those working within it, or you can call 0800 022 3818. Both services enable you to leave details of who you are, which will then be kept confidential within your FRS - or you can stay 100% anonymous and no one will ever know who you are, not even us.

Talking to one of our independent call agents or filling in this form will enable you to pass on what you know. Be sure to give as much detail as you are able to, as this helps build the full picture and ensures that the best use can be made of your information.

Phone calls are not recorded or traced, and all information submitted online is via an encrypted connection.
